What to Expect at Cobblestone

Types of Admission

Private Pay Long Term Admission : (For permanent placement)

Before a long term admission to Cobblestone, certain information is required to ensure that Cobblestone is able to care for all the needs of a potential Member. Below is a list of requested information. This list is not comprehensive and additional information may be required on a case by case basis:

  • Complete History and Physical from current primary care physician
  • Recent TB test or Chest x-ray
  • Immunizations
  • Current medication list
  • Pertinent lab work
  • Confidential data application
  • Advance Directives (i.e.; Living Will, Healthcare Power of Attorney, Financial Power of Attorney, Do Not Resuscitate Order, etc.)

Long term admissions can be paid for privately or through the use of long term care insurance. Cobblestone at Park Springs will work with you to gather any and all requested information as needed by your long term care insurance company.

Short Term/Sub-Acute/Rehabilitation Stays: (For temporary placement)

Many Members admit to Cobblestone for a short term stay after a brief hospitalization for sub-acute rehab/therapy services. This is typically coordinated through the hospital social worker and the Cobblestone admissions team. The hospital social worker will fax clinical information that will be reviewed by our interdisciplinary team to ensure that your care needs can be met at our facility. We will also verify insurance coverage to guarantee that your services will be covered. If you have Advance Directives, we will also request a copy of those. If you do not have Advance Directives, our Social Services Team can assist you with developing them.

Short term stays are usually covered by Medicare Part A or private insurance. We will verify your insurance benefits and eligibility prior to admission.

Respite Stays

Respite stays are also permitted at Cobbletsone. A respite stay is a short term stay that is intended to provide caregiver relief. Respite stays are a minimum of 5 days in length. The required documents are the same as those outlined for a long term stay:

  • Complete History and Physical from current primary care physician
  • Recent TB test or Chest x-ray
  • Immunizations
  • Current medication list
  • Pertinent lab work
  • Confidential data application
  • Advance Directives (i.e.; Living Will, Healthcare Power of Attorney, Financial Power of Attorney, Do Not Resuscitate Order, etc.)
